In this video, Dr. Nicole Alonzo, a vascular surgeon at New York Presbyterian Brooklyn Methodist Hospital and chief of clinical research, discusses her experience using the Inari Indril thrombectomy sheath.

Dr. Alonzo had a patient who was unable to have TPA, a common treatment for clotted AV fistulas and grafts. This patient needed a D-clot to avoid becoming catheter dependent. Dr. Alonzo decided to try the Inthrill device, which requires zero TPA and effectively restores access for dialysis patients. This eliminates the risk of intracranial hemorrhage or bleeding from a surgical site, as TPA is not used.

The Inthrill device has a nitinol core that can mechanically disrupt even subacute clot, making it more efficient than the traditional balloon emaceration technique. On average, the procedure time is only 45 minutes and has been successfully used on over 80 patients. It is also used in Dr. Alonzo’s office for patients who she has created fistulas or inserted AV grafts for, as these patients have a 22% chance of thrombosis within two years. In a retrospective study, 95% of patients who underwent thrombectomy with Indril had AV grafts and had a 77% primary patency rate, with 100% secondary and assisted patency. The average procedure time was 45 minutes with minimal blood loss of only 10 cc’s.

Dr. Alonzo has been involved in research for over 15 years and has published over 50 peer-reviewed publications, including five with Inari. She has seen firsthand the success of the Indril device in maintaining access for dialysis patients. With this device, clinicians can effectively remove thrombus in a minimally invasive and safe manner, without the use of TPA. Complications are minimal and 95% of patients are able to use their access the same day.
